Again, I really have only one main image that I remember from growing up...
It ends with a young boy and his babysitter/foster mum (played by a large woman who I THINK I recognize but until I find out her name am not sure) who are sitting in a small wooden boat on a dark lake, and the large woman has a massive asthma attack, causing the boat to tip over, spilling them both into the water.
The boy swims to shore, but it ends on an ambiguous note as to weather the woman survived or not.
When I saw it when I was little I remember screaming and crying in shock and my mum had to comfort me :P
I THINK it was a UK one-off TV drama, but really, the only bit I remember is what I described above about the ending...
